New Clients
Please be ready to provide us with your insurance information, deductible amounts, co-payment amount and any pre-authorization number(s), if needed, that you obtain by calling your behavioral health insurance company or Employee Assistance Program when contacting us to schedule your first appointment. We will only be able to see clients who have provided us with this information prior to their initial session.
Client Intake and Assessment Forms
Once you have scheduled your intake appointment, please go to our Patient Portal at Patient Ally www.patientally.com and use the log in information provided via email to complete the new client intake forms prior to attending your first appointment.
The Patient Portal integrates with www.patientally.com, our Free patient health record
portal. Patient Ally is a communication portal allowing patients and providers to communicate with
each other. Patients using Patient Ally can request appointments, send secure messages, and submit forms/documents.
